Invalid callout title on nested callouts with links and footnotes

Describe the bug
The nested callouts can have invalid titles when the nested callout's content has a link or a link to a footnote.

To Reproduce
Render this in quartz:

> [!abstract] Outer
> [[Link]]
> > [!error] Inner
> > [[Link]]

Expected behavior
